試管嬰兒是在體外進行胚胎的培養,培養的環境是否能有助於胚胎的發育,對試管嬰兒療程的成功率是相當重要的。相較於傳統的濕式培養箱,新一代的乾式培養系統有降低交叉汙染的機率、開關門後氣體濃度回升迅速和更穩定的溫度控制等等的優點;而從經濟面的角度來看,乾式培養系統體積小、省空間、氣體消耗量少,正可謂經濟又實惠。乾式培養系統的運用在國外雖然已經有不錯的成績發表,但目前在台灣還不是主流的胚胎培養系統,送子鳥在2011年引進乾式培養系統(IVF cube, ASTEC, Japan),並將使用經驗與成果發表在2012年台灣生殖醫學年會上。

研究設計

卵子以自然受精或顯微受精方式進行受精,再分別觀察

》受精率:培養第1天,形成兩個原核細胞(2PN)的比率

》好胚胎率:培養第3天,分裂成6細胞以上且細胞碎片小於20%的比率

》好囊胚率:培養第5天,成長為6AA~3BC (Gardner grading)等級的囊胚比率

研究結果

》週期數:49

》平均年齡:38.3歲(30-49歲)

》成熟卵子(MII oocyte):183顆

統計結果如下圖,使用乾式培養系統的受精率為80.3%、好胚胎率為42.1%、囊胚率為47.0%、好囊胚率為32.8%,其中有69.8%囊胚是被歸類為好囊胚的等級。

結論

結果顯示,使用乾式培養系統培養胚胎的成績並不亞於傳統的濕式培養箱,加上前述的各項優點,乾式培養系統的確有其實際的臨床用途。

The advantages of dry culture system include minimal risk of contamination, fast recovery of gas concentration, and more stable temperature controlling. From economical point of view, dry incubators are space and gas saving due to small chamber volume. However, there are disadvantages such as swift variation of environment and higher water evaporation rate. Dry incubators are not as common as humidified incubators in routine use. To assess the clinical application, a preliminary feasibility study was conducted.

Average maternal age: 38.3 y/o (30~49)

Total MII oocyte: 183

Conclusion

These preliminary results indicated that the dry culture system for human embryos was practical. Further research would be performed to compare the efficiency of dry incubators with humidified incubators.
